For use by a contract-holder following receipt of a notice of rent variation under section 104 or 123 of the Renting Homes (Wales) Act 2016. The occupation contract must be a relevant converted contract within the meaning of paragraph 15(3) of Schedule 12 to that Act. (Application form RAC4)
